hi PRE-SEASON SPORTS PHYSICAL Schedule your sports physical today! (2025-2026 School Year) Call DMHC Clinic at 487-2296. • SPORTS PHYSICAL examination meant to assess immediate fitness levels and ensure it's safe to engage in sports or strenuous activities. • $40.00 pay ment must be made on the day of your appointment. • Forms must be completed and signed by parent/ guardian prior to your appointment. • WELL CHILD VISITS - regular examination of your child's overall health, physical, emotional, and social growth. Sports clearance will be done during this exam. • Most health insurance plans cover well child visits without cost-sharing, please review your benefits to avoid extra cost if you choose to submit to insurance. • MHSA has updated their sports physical exam, requiring them to be completed every 2 years.
